Explicitly Queer Manga [SPOILERS]
MangaSpoiler
Manga that I've read that center around queer characters and queer problems, not just BL or GL. Although some of these manga might not exactly fit these criteria I still highly recommend all the manga I have listed here.
Some descriptions might be short or are undetailed because I haven't read some of these in a while, will update when I do.

Multiple gay, bi, and trans characters. Has an anime, but is incomplete.
This manga might not exactly fit this stack. Rei Oohashi (now Rae Taylor) gets isekai'd into an otome game (a type of dating simulator targeted towards women). Instead of going for one of the romanceable men in the game, she instead decides to go for the rival character, Claire Francois.
This manga is much more than just about queer topics, but I believe when it does, it handles them well. If there is one caveat some people may dislike (myself included) the manga compares queer relationships to an incestuous relationship two characters have, thankfully this is brief and I believe does not take away from the overall enjoyment of reading this manga.

A manga about a mother realizing that her son might be gay. The story centers around her encouraging her son as she tries to understand the perspectives of queer people.

Lesbian main characters whom have to navigate through their relationship and their traumas. This manga is much harder for me to describe unlike others as it is still on-going and is much more than its queer narrative. In its essence it is exactly as the title says, it is about relationships itself.
Queer or not, if you like realistic takes on relationships, read this manga. I highly recommend it.

This manga is the start of a series of autobiographical manga by Nagata Kabi, its mangaka. This one specifically is mostly about her self-discovery and realization of many of her traumas and coping mechanisms.

The main character gets introduced to his late brother's husband, and he tries to understand and move past his previous prejudices about gay people.
The mangaka adds information about certain queer identities as little factoids at the start of certain chapters called 'Mike's Gay Culture Lectures' that I find are informative and fairly accurate despite the age of the manga.
If there's a manga on this stack that I would choose for you to read, it would be this one.

Centers around 2 transgender* main characters. Disappointingly, the transmasc character turns out to 'not be trans' by the end of the manga. Has an anime adaptation, but is incomplete.

Main character gets their sex swapped. I may or may not remove this one as this one might not exactly fit the stack, but I still highly recommend it.
The manga centers around Akira, once male turned female by a disease. As this manga is still ongoing I cannot give an accurate review of how it handles its queer subjects.

Main character's love interest is genderqueer/demigender and the story centers around this.
This manga handles sexual assault and domestic abuse, if you feel uncomfortable with these topics do not read this manga.
The manga itself does not use any language to describe its main characters.

Like a lot of Oshimi's works it handles gender identity, gender expression, and sexuality. This one also might not exactly fit the stack, but I highly recommend this one.

An autobiographical manga about the mangaka who is a transgender woman.
The manga features fairly decent information (although a little outdated) about queer topics.

Rika is a transgender man, and is the main character. A highlight of this manga is the struggle of the secondary main character (Satsuki) after realizing he is attracted to Rika 'as a woman'.
With the manga only being 13 chapters, the ending may feel as though it leaves many things unresolved.
From what I can tell this manga does not use any outdated language of any kind and is decently accurate in its representation.

This manga centers around the relationship of Mine-kun (asexual) and his girlfriend, Murai. This one has a 'sad' ending.
This one-shot has a small moment where a character invalidates Mine-kun's sexual orientation.
As I am not asexual myself I cannot tell too well if this manga accurately describes asexuality, but from what I've read it has a fairly accurate depiction.

A manga that tackles self-discovery, queerness, and societal expectations of women. So far a great manga, a great balance of fluff and serious moments.
